Include: Name, Age and height /weight or everyone being submitting. Include # and city/state of residency. Include a head shot and full body of each individual and if you are submitting more than one person include a group photo along with the head/body shots.
**NOTE** if your kid/kids get selected you must be okay filling out a minor consent form and submitting a copy of their social security card and birth certificate.
